Request Direct Deposit Authorization You may request Direct Deposit Authorization of your commissions by completing the Request Direct Deposit From on-line.  If you already have your commissions directly deposited, but would like to change information regarding your accounts or cancel your direct deposit election, please complete the Request Direct Deposit Authorization Form again with the new information.
